Registration forms for Winter 2011-12

Winter activities registration begins on Wednesday, November 16, 2011 at 8:30am for everyone. Winter classes begin on Monday, December 12, 2011. Registration forms can be obtained by clicking on the links below. Detailed descriptions of the winter classes can be found here.

Please Note: Tuesday Fashion Diva Classes will end on February 14th due to instructor absence earlier in the term.

New Emergency Contact Forms, Afterschool Participation Fee Forms and Release Forms are required from all participants each school year. They can be downloaded in Adobe Acrobat format by clicking the name of the form.

Purchase School has arranged a bus service, through J & R Tours, that runs from Purchase School to the Purchase Community House after school. Click here for a 2011-12 bus registration form and more information about this bus.
